ABSTRACT

El sialocele es una colección de saliva en los tejidos que rodean al conducto de la glándula o del parénquima sin un drenaje adecuado. Su causa más común es la extravasación de saliva producto a una disrupción del parénquima o conducto parotídeo secundario a un trauma cortante. Si el tratamiento del sialocele no se realiza en forma oportuna se puede generar una fístula externa, cicatrices faciales e infecciones secundarias. Un hombre de 24 años ingresado al Hospital con múltiples fracturas faciales sin trauma cortante. Luego de la disminución del edema, continuó el aumento de volumen en la región geniana en forma localizada, fluctuante, ovalada. La piel se encontraba distendida, asintomática, sin secreciones, con alteración de la función motora y sensorial en la región geniana derecha. Se realizó la aspiración del contenido del aumento de volumen. Después de 2 días recidivó. Se solicitó una Tomografía computarizada y se volvió a realizar aspiración del contenido para enviarlo a cultivo citológico. Se hizo el diagnóstico de sialocele post-trauma de la parótida y se realizó un vaciamiento del contenido y drenaje tipo penrose intraoral. Se controló al siguiente día sin recidivas y se retiró el drenaje a los 2 meses. Es importante tener en cuenta que se puede generar un sialocele post-trauma sin necesariamente ser cortante.


Sialocele is a collection of saliva in the tissues surrounding the duct of the gland or parenchyma without proper drainage. The most common cause is the extravasation of saliva product to a disruption of the parenchyma or parotid duct secondary to a cutting trauma. If sialocele treatment is not performed in a timely manner it can generate an external fistula, facial scars and secondary infections. Male admitted to hospital with multiple facial fractures. After the reduction of edema, continued increased volume in the preauricular region localized, fluctuating, oval. The skin was asymptomatic, without secretions, with impairment of motor and sensory function in the right genial region. The aspiration of the increase in volume under a hypothesized hematoma was performed. After 2 days recurred. A tomography scan was requested and returned to realize the aspiration of which was sent to cytological exam. The diagnosis was post-trauma sialocele parotid and was performed emptying the contents and drainage type penrose intraoral. Was control the next day without relapses and the drain was removed after 2 months. It ́s important to know that can generate a post-trauma sialocele without necessarily cutting trauma.

ABSTRACT

Childhood parotid swelling has a number of differential diagnosis mostly of inflammatory origin. Pneumoparotitis is an uncommon cause of parotid inflammation. It is caused by an excessive increase of intraoral pressure and secondary passage of air into the Stensen or Stenon duct and its glandular branches. Diagnostic clues can usually be obtained by a directed anamnesis. Ultrasonography (US) and computed tomography are essential diagnostic tools for this condition that has a benign course with spontaneous resolution in most cases. We present four cases of pneumoparotitis diagnosed by US in children 5 to 13 years of age. One of the cases occurred after the child chewed gum and made bubbles for a prolonged timeperiod and the other three after inflating baloons, making bubbles inside a pool and after playing the flute. All cases resolved spontaneously after two days. We suggest to consider pneumoparotitis in the differential diagnosis of parotid swellig in children.

ABSTRACT

This case report documents one of the more unusual causes of a facial swelling in the preauricular and buccal region, post-traumatic parotid sialocele. Facial lacerations are common injuries; however, parotid gland involvement and, in particular, ductal transection is relatively uncommon and only 0.2% of such patients have a parotid gland injury. A sialocele typically develops 8 to 14 days after the injury but in our case, the presentation was delayed [8 months after trauma]. Sialography can play a significatnt role in the diagnosis of sialocele by indicating the extent of parotid duct injury. We describe a 17'year' old girl with progressive marked facial swelling in the left parotid and buccal area with past history of penetrating facial injury. Using text and images, we detail our diagnostic management. This case report illustrates the relationship of trauma to sialocele formation, while suggesting that sialography shoud be done in oral, neck and face masses suspected to be related to salivary ducts. Posttraumatic parotid sialocele should be considered in the differential diagnosis for any posttraumatic facial or high neck swelling

Apresenta-se um caso de lesäo do conduto de Stenon, como complicaçäo de exérese de cisto hemifacial direita. Esta lesäo, representada por fístula salivar externa, decorreu de incisäo vertical na regiäo bucinadora. O tratamento empregado consistiu na dissecçäo e canulizaçäo do conduto de Stenon, sendo reimplantado na mucosa bucal, atavés da técnica de von Langenbeck modificada, obtendo-se a cura do processo fistuloso
